Post Stroke Care

Patients with dementia experience a decline in cognitive abilities such as language judgment, judgment, memory, orientation, and calculation. This decline often leads to challenges in their daily lives. It's crucial to modify their living environment, minimize confusion, utilize environmental cues and stimuli, and support them in maximizing their capacities to maintain a fulfilling and dignified quality of life.

​Service Target :

Post-stroke patients/patients with symptoms of stroke


Includes physical therapy, occupational therapy, speech therapy and other post-stroke care.


Care plan:
The therapist will assess the patient's condition and develop an appropriate treatment plan, during which the caregiver will assist with daily living.

Physical Therapist (PT)

Physical therapy aids in the restoration of movement and function for individuals affected by injury, illness, or disability. It not only facilitates recovery but also plays a crucial role in minimizing the risk of future injuries or illnesses.

Occupational Therapist (OT)

Occupational therapists specialize in treating individuals who are injured, ill, or disabled, using everyday activities as therapeutic tools. Their goal is to assist patients in developing, recovering, improving, and maintaining the skills necessary for daily living and work.

Speech Therapist (ST)

Speech pathologists focus on studying, diagnosing, and treating communication disorders, addressing challenges in speaking, listening, language comprehension, reading, writing, social skills, stuttering, and voice use. Speech therapy also provides crucial support for individuals dealing with swallowing difficulties and various communication disorders.

The Personal Care Worker (PCW) plays a crucial role, encompassing responsibilities such as:

Monitoring and communicating the individual's condition

Escorting the patient to hospitals and clinics

Assisting with personal hygiene and showering

Offering meals and facilitating other health and wellness activities.

Health Worker (HW) Services:

In addition to the services offered by Personal Care Workers, Health Workers provide specialized professional care, including wound care, continence & catheter support, diabetes care, colostomy care, and dementia care.
